The Women, Infants, and Children (WIC) program supports mothers and young children by providing nutrition education, breastfeeding support, healthy food assistance, and connections to healthcare and community resources.
Ryan White Program
The Ryan White program provides care for individuals living with or at risk of HIV, including testing, treatment, prevention services, and ongoing support to help patients manage their health and well-being
Cancer Prevention Program
Our Cancer Team supports patients through cancer prevention, screening, care coordination, and access to treatment, helping guide them through every step of their care.
Diabetes Program
Our Diabetes Team helps patients prevent and manage diabetes through personalized care, education, nutrition guidance, and ongoing support to improve overall health.
They will be giving blood pressure checks at their table!
The NIMAA program at Fair Haven Community Health Care is an affordable, hybrid training program that prepares students to become certified medical assistants through a combination of online coursework and hands-on clinical experience in a real healthcare setting.
Our Food as Medicine program helps patients improve their health by providing free access to fresh, nutritious foods through a “Food Farmacy,” along with nutrition education, cooking workshops, and support for managing food insecurity.
They will be hosting food demos of Spring favorites throughout the event!
Our Dental program will be providing information on FHCHC's Dental services and Oral Cancer information.
Community Partners
BHcare provides trauma‑informed mental health, substance use, and domestic violence support services to empower women and promote wellness and safety.
CRIW empowers women through community-driven programs in education, arts, and advocacy to promote wellness, leadership, and self-determination.
DESK provides individuals experiencing homelessness or living in poverty in New Haven with daily meals, food pantry access, a drop‑in resource center, and supportive services that promote health, community, and equity.
Havenly builds the community power of women in New Haven by offering job training, education, and paid fellowship opportunities through its community café and support programs that promote economic and civic empowerment.
Junta for Progressive Action empowers Hispanic individuals and families in Greater New Haven with social services, education, advocacy, and leadership programs to advance social, economic, and civic well‑being.
The Mammo‑Van brings state‑of‑the‑art breast cancer screening, including 3D mammograms and ultrasounds, directly into Connecticut communities. Make your appointment today!
Project Access increases access to medical care and services for underserved individuals in the Greater New Haven area.
The Connecticut WISEWOMAN Program serves about 1,200 Connecticut residents every year and have helped around 4,500 people visit doctors, get cardiovascular screenings, access nutrition and physical activity interventions, and more.
Yale Cancer Center advances cancer research and patient care through innovative treatments, prevention, and multidisciplinary support.
