Whether you live, work, attend school, own a business, receive services, or have other connections in Greater New Haven or the Connecticut Shoreline we want to hear from you!
Your feedback will help us better understand the health needs of our communities, identify barriers to accessing care, and recognize the resources and programs that are making a difference. What you share will help guide our efforts to improve health care services and build healthier communities.
The survey is anonymous and confidential. It asks about community needs, access to health care, and the resources people rely on. Optional demographic questions help us understand whose voices are being represented.
