At our medical satellite at Bella Vista housing complex, the Fair Haven Medical Group at Bella Vista brings health care literally to the doorstep of the residents. With continued funding from the State Department on Aging and the City of New Haven, our program helps to support people to live independently in their own apartments because health care is easily accessible.

The Medical Group's offices are staffed full time by a Site Manager who is a nurse practitioner, an office manager, a nurse, and a clinical assistant. In addition to the nurse practitioner, the medical care is provided by the internists who travel to Bella Vista from the Main Clinic. We have recently expanded our services to include a consulting psychiatrist. As with all our primary care services, each patient selects a primary care physician.

 

School Based Health Centers (SBHCs)

We are pioneers in bringing health care directly to teens. We began with The Body Shop at Wilbur Cross High School in 1982 and have developed a range of other successful programs that serve this challenging population. The Body Shop provides a range of health services, social services and special education programs on anger prevention, substance abuse, HIV, fitness and social development.

Nurse practitioners with consulting pediatricians, receptionists, health educators and social workers, provide care to all registered students at each school. We require a completed permission slip signed by a parent or guardian. In several of the schools we collaborate with counselors from the Clifford Beers Child Guidance. The State of Connecticut , Department of Health through the New Haven Board of Education, provides support for the SBHCs.
