Choosing a Community Health Center

You join a healthcare team when you choose to receive your healthcare at FHCHC.  We work together in partnership to provide you with the high quality healthcare you and your family needs. When you decide to join our team, you can choose your Primary Care Provider (PCP) and a team of healthcare providers who work best with you. Your team will help you coordinate care across many healthcare settings such as hospitals and specialty care providers. We are committed to providing the care and support you need when you need it, and helping you learn how to manage your own care. Learn about participating in our model of healthcare--Patient Centered Medical Home. Please call our appointments department to make an initial appointment with one of our healthcare providers.
