Haven Community Health Center (FHCHC) provides accessible
and comprehensive health care in New Haven, CT.
The
Fair Haven Community Health Center (FHCHC) is a not-for-profit
primary health care organization dedicated to serving the
greater Fair Haven Community. We provide comprehensive health
care – from prenatal to pediatric, adolescent to adult
and geriatric. Our mission is to provide excellent, accessible
health care to the residents of our community, regardless
of their ability to pay. A consumer and community-based
board of directors, the majority of whom are patients, gives
guidance and direction to our mission.
FHCHC is a federally
recognized community health center, and has been nationally
recognized as a leader in health disparities collaboratives
and chronic disease management. Our clinical staff includes
board certified physicians, pediatric and family nurse practitioners,
physician assistants and certified midwives. We offer convenient
social service and behavioral health counseling, on-site
lab services, WIC program and wellness activities. In
addition to a full range of primary care services, deliveries
of our pregnant patients in the hospital, and care of our
patients when then they are hospitalized, we offer convenient
social service and behavioral health counseling, on-site
lab services, a WIC program and wellness activities.
24 Hour phone coverage
is provided for patients by our clinicians through our answering
service.
FHCHC is affiliated
with the Yale-New Haven Hospital and the Hospital of St.
Raphael for hospitalizations, diagnostic testing and specialty
referrals.
The Center is not
a walk-in center and encourages patients to call ahead for
an appointment. We make every attempt to see established
patients for episodic problems within 24 to 48 hours depending
on the severity of the problem.
The Center accepts
new patients who live in the Fair Haven and Wooster Square
Neighborhoods.
The Center participates
in most commercial insurance plans, including Anthem Blue
Cross, ConnectiCare, Oxford , Cigna, HealthNet etc. In addition,
the Center accepts Title XIX, and SAGA and participates
in Community Health Network, Preferred One, and HealthNet for patients
with HUSKY coverage.
The Center serves
uninsured and underinsured patients on a sliding fee scale,
depending on the family's income and how many people are
in the family.
