Chief Executive Officer
Suzanne Lagarde MD, MBA, FACP is a graduate of Cornell University Medical College. She completed her residency and GI fellowship at Yale New Haven Hospital and before joining Fair Haven Community Health Center, she spent her professional career in the private practice of gastroenterology in southern CT. Dr. Lagarde is a founding member of Project Access-New Haven (PA-NH) and currently serves as immediate past President of the Board of Directors.
Dr. Lagarde is also licensed in Mississippi and until recently, she made quarterly trips to Biloxi since 2007. When in Mississippi, she worked at Coastal Family Health Center, a clinic serving over 30,000 patients, most of whom are uninsured. In April 2010, Dr. Lagarde received the Biloxi Health Volunteer of the Year Award.
In CT, Dr. Lagarde works on several initiatives aimed at increasing access to screening colonoscopy for uninsured patients. In collaboration with Yale-New Haven Hospital, she has established a program of providing free colonoscopy to 120 patients yearly. She also serves on the Medical Advisory Board of the CT Colon Cancer Control Program, which provides approximately 500 uninsured CT residents per year with free screening.
Dr. Lagarde also serves as head of medical triage of CT Mission of Mercy, an annual 2-day event that provides free dental care to over 2000 patients. She is a Global Medical Advisor to AmeriCares and serves on the Ethics Committee of Yale-New Haven Hospital. She was the 2010 recipient of the CT Chapter of the American College of Physicians Volunteerism and Community Service Award and was named a Community Hero by the New Haven Chapter of the American Red Cross in 2011.
Senior Vice President
Ms. de Jesús served as a special advisor to Governor Dannel P. Malloy on implementation of federal health care reform in CT, deputy commissioner of the Department of Public Health, senior vice president of Healthcare Integration at Wheeler Clinic, vice president of the CT Hospital Association and CEO of the Hispanic Health Council. She has a Master of Social Work degree from New York University and a Master of Public Administration from Harvard University. She lives with her family in West Hartford.
Compliance Officer and QI Liaison
Ms. Hoffmann serves as the Health Center’s Corporate Compliance Officer and QI Liaison. In this capacity, she is responsible for the development and oversight of Fair Haven’s Corporate Compliance Work Plan and for chairing its Corporate Compliance Committee. She collaborates actively with all staff, clinicians, administrators and Board members to ensure that Fair Haven promotes an on-going culture of compliance and quality.
A graduate of the University of Louisville and the Yale University School of Nursing, Ms. Hoffmann has over 25 years of multifaceted experience in quality improvement, regulatory and accreditation compliance, and administration within ambulatory health care, Medicaid managed care, and State Medicaid Agency settings. She maintains active certification in health care compliance from the Health Care Compliance Association (HCCA). She and her husband live in the City of Groton, where they have been active in community revitalization and historic preservation efforts.
Vice President of Finance
Vice President of Clinical Affairs
.A native of Boston, Dr. Olson graduated from George Washington University School of Medicine in Washington, DC. He completed residency and chief residency in internal medicine/primary care at Yale. He is board certified in both internal medicine and addiction medicine. Dr. Olson is responsible for clinical integration among medical, behavioral health, addiction medicine and dental programs at FHCHC.
Since being an intern for Senator Edward Kennedy as a college student, he has had a firm commitment to health equity and eliminating health disparities. He works to create and improve clinical-community partnerships that positively impact all patients and staff. He is active in health reform and advocacy, serving on the State of CT Practice Transformation Task Force and also on the State of CT Medical Examining Board.
He lives in Cheshire, CT with his husband, Justin, and 2 dogs, Roxy and Kane
Vice President of Programs
Ms. Paine is a graduate of Oberlin College, and holds Master’s degrees from the School of Social Work and the Mailman School of Public Health at Columbia University. She has worked at Fair Haven Community Health Center (FHCHC) since 2007, where she is currently the Director of Programs. She is responsible for the oversight of all programs at the health center, writes grant applications, and oversees all grant-related functions. Ms. Paine also leads quality improvement initiatives, including:leading the health center’s Quality Improvement Council; and collecting and reporting data on UDS clinical quality measures.
Prior to joining FHCHC, Abigail worked at the Boston Public Health Commission, in the AIDS Program where she provided technical support to community health centers and other organizations receiving funds through the Ryan White Part A program. She began her public health career as a medical assistant in a Planned Parenthood clinic in Chicago, IL, where she learned firsthand how access to health care can improve people’s lives.
Abigail was born and raised in New Haven, is a graduate of Wilbur Cross High School, and currently lives here with her husband and three children.
