Behavioral Health Clinician

Responsible for providing mental and behavioral health evaluations, diagnosis, treatment planning, individual and group therapy, and (if appropriate) brief substance abuse counseling for patients of Fair Haven Community Health Center.

Clinical Assistant

Clinical Assistants work with clinicians and nurses to provide high quality, cost effective medical care to patients in a medical home that is consistent with Fair Haven Community Health Center (FHCHC) policies and missions.

Dental Hygienist (part time)

Responsible for the administrative oversight of HIV medical and support services ensuring that the program meets Ryan White and other funding requirements. Duties include grants administration and reporting, shared supervision of program staff and collaboration with community agencies to improve services and access to care for HIV affected patients.

Interpreter

The Interpreter functions as part of a health care team, meeting the needs of patients, staff and consumers by facilitating the needs of Spanish speaking patient with limited English proficiency (LEP) and others. The interpreting services coordinator assists patients, consumers and staff with any interpreter requests to provide meaningful access for individuals with LEP and promote the most efficient, professional and friendly access to our facilities.

Nurse, HIV and Infectious Disease Program

To enhance the health and wellness of HIV-positive and LGBTQ individuals in our community by providing affirmative, high quality health care, patient education, support and advocacy. To provide health education and support to patients to maintain health and wellness, manage acute and chronic illness, and adhere to medications and treatment plans. To provide clinical support to Medical Director of HIV and Infectious Disease Program.

Responsible for providing psychiatric evaluations, diagnosis, treatment planning, and medication management at FHCHC, a federally qualified healthcare center offering integrated outpatient healthcare. Responsible for providing supervision to behavioral health (BH) clinicians, APRNs and residents/medical students if/when asked to do so.
