Behavioral Health

The Behavioral Health Department at Fair Haven Community Health Center believes integrated behavioral healthcare is essential for high quality primary care.  We offer psychiatric evaluations, medication management, short-term individual therapy, psychotherapy groups, and in school counseling services. The majority of our behavioral health clinicians speak English and Spanish to meet the needs of our patients and their families.

Our goal in the Behavioral Health Department is to coordinate care with FHCHC’s primary care providers to provide high-quality healthcare. Currently, all adult and adolescent patients at FHCHC are screened for depression during their primary healthcare visits and referred to behavioral health services if needed. Adult patients are screened for substance abuse following the SBIRT model (Screening, Brief Intervention, and Referral to Treatment) by Integrated Behavioral Health Clinicians.Our behavioral health patient navigators work to integrate care on the medical floors by connecting our patients to the behavioral health department during their primary care visits.

Wrap Around New Haven

Fair Haven Community Health Center and FHCHC’s School Based Health Centers are excited to partner with Clifford Beers Clinic to implement an integrated behavioral health initiative called WrapAround New Haven.  The initiative’s aim is to improve health outcomes in children and adolescents with chronic medical and behavioral health conditions who have been seen in an emergency room multiple times by linking community-based care coordinators with these children and their families.  Children and their families are identified by their primary care provider at FHCHC or one of our SBHCs then referred to a Clifford Beer’s care coordinator. Care plans for each family are created using relevant medical history and behavioral health screenings, and the care coordinators will help the families manage the transitions between healthcare providers as well as offering to help connect them to appropriate services.  Read more here..
