In addition to our main site, Fair Haven Community Health Center has extended its reach into the community by establishing sites at several schools and the housing complex for the elderly, Bella Vista. The satellite sites allow us to provide high quality convenient care for children, teens, and the elderly who cannot always attend appointments at the clinic’s main site. Our staff caters to the individual needs of the population they serve to promote comprehensive primary health care.  As a result, we are able to address a wide range of health issues throughout the Fair Haven community.

At our medical satellite at Bella Vista housing complex, the Fair Haven Medical Group at Bella Vista brings health care literally to the doorstep of the residents. With continued funding from the State Department on Aging and the City of New Haven, our program helps to support people to live independently in their own apartments because health care is easily accessible.

The Medical Group’s offices are staffed by a full time Office Manager, Nurse Manager, and a Clinical Assistant.  Additionally, medical care is provided by internists who travel to Bella Vista from Fair Haven’s main site.  Our services also include a consulting psychiatrist.  Each patient is assigned a primary care physician.

We are pioneers in bringing health care directly to teens. We began with The Body Shop at Wilbur Cross High School in 1982 and have developed a range of other successful programs that serve this at-risk population. We now have School Based Health Centers at five schools in New Haven.

The Body Shops provide complete medical services including physical exams and immunizations, through the care of Advanced Practice Registered Nurses, Physician Assistants, and Pediatricians.  Licensed Clinical Social Workers as well as our partnership with the Clifford Beers Consultation Guidance Center provides support for students with emotional and social needs.  We also offer the services of substance abuse counselors and midwives as well as a pediatrician once a week at each site.  Our office managers help students schedule appointments to ensure access to healthcare.  The Body Shop staff work together to conduct support groups for students in the areas of self-esteem, nutrition and fitness, substance use, bereavement, and HIV/AIDS.

We require a completed permission form signed by a parent or guardian.  The State of Connecticut, Department of Health through the New Haven Board of Education, provides support for The Body Shops.
