Clinicians at Fair Haven Community Health Center provide comprehensive primary health care for children and adults, including specialized care for chronic conditions like diabetes, high blood pressure and asthma.  Additionally, our team of midwives provide prenatal and Well Baby Care. We are not a walk-in health center so please call to make an appointment. For answers to frequently asked questions, see below.

To make an appointment with a clinician at Fair Haven, please call 203-777-7411.  Please be prepared to provide your name, date of birth, address, phone number, and insurance information.  You must be a resident of Fair Haven (zip code 06513) to be a patient.  Also, a clinician at Fair Haven must be designated as your Primary Care Provider (PCP) with your insurance company.  New patient appointments are generally available within 6 to 8 weeks.

If you need to cancel an appointment, please call the health center (203-777-7411) as soon as possible.  If you miss an appointment, you may call the clinic to reschedule.

When you come to your appointment, please bring your immunization records, any medications you are taking, your insurance card or proof of income, and any other important health information you have.

In case of emergency, please dial the main clinic number 203-777-7411.  During business hours, a nurse or clinician will arrange an appointment for the same or next day.  After business hours, you will be directed to the answering service, which provides 24-hour telephone coverage to our registered patients.  Please leave your name, clinic number, and telephone number with the Answering Service.  They will call the clinician on call.  Please call the Answering Service back if a clinician does not return your call within 20 minutes.  Please dial 911 in case of a life threatening emergency or proceed to the nearest Emergency Room

Hospitalization

In the event that you are hospitalized, our clinicians will monitor your care at the Yale New Haven Hospital.  Our patients’ babies are delivered at the Hospital of Saint Raphael.

Requesting medical records

To have your medical records sent to another facility, please complete an Authorization for Release of Information form available at the clinic.  Please allow 7-10 business days to complete the request.

Refills

Please contact your pharmacy for medication refills.  The pharmacy will contact your clinician at the clinic to obtain authorization.  Please allow 24 to 48 hours for authorization.  You may also notify our clinical staff about your refills during an appointment.

Payment

FHCHC accepts many insurances.  Please bring your insurance card to all of your appointments. 
Uninsured patients are billed on a sliding fee scale based on their family size and income.  Please bring one of the following proofs of income to your appointment :
- Two of your most recent pay stubs
- W-2 or a 1040 Tax Return form
- A  letter from your employer with your current gross income 
- Unemployment benefit notification

All payments are due at the time of visit.  Payment plans can be arranged by our billing staff upon request.

Making appointments at Bella Vista

The Fair Haven Community Health Center has a satellite site at the Bella Vista housing complex to serve residents only.  If you are a resident at Bella Vista, you may call 203-469-5331 to make an appointment with a provider.

At the high schools, students may walk in to the School Based Health Centers (SBHC) with a hall pass when they need to see a provider. Students in elementary and middle school are referred to the SBHCs by a teacher or counselor.
